Please send all the documents below to the school you signed a contract with.   1. University diploma (a notarized and Apostilled diploma ONLY) First, have a copy of your diploma notarized by a notary public. Second, attach an Apostille Certificate to the notarized copy at the Minnesota Secretary of State.   If you have just graduated and want to go to Korea as soon as possible, you need to go to the registration office to 1) get a letter of graduation (a graduation certificate), 2) get it notarized and then 3) attach an Apostille Certificate to it at Minnesota Secretary of State - Certification, 60 Empire Drive, Suite 100, St. Paul, MN 55103-2141 (www.sos.state.mn.us).   2. 4 passport photos (2 x 2 inches in size and in full color with a pure white background)   3. 1 photocopy of your passport’s personal data pages (pages with photo & signature).   4. Your resume.   5. 1 signed copy of contract.   6. Completed Applicant’s health statement (Download it from our website).   7. Criminal Record Check: Only Apostilled FBI Criminal Record Check will be accepted!   Please use Express mail when you send in application and enclose a pre-paid and self-addressed Express mail envelope for faster and safer return.    First, apply for FBI check. You MUST write a note in the packet that you want to get your check apostilled. This MUST be requested at the time of FBI check request. Visit FBI website: http://www.fbi.gov/about-us/cjis/background-checks/background_checks    Second, you need to send the FBI check to the Department of State to attach Apostille. Visit U.S. Department of State website: www.state.gov/m/a/auth/index.htm   NOTE :  All the documents must be less than 6 months old.  Your passport must not expire within 6 months.  Your full name on your passport must exactly match with that on other documents.  Please use Global Express Guaranteed or FedEx International Priority when you send the first set of visa documents to your school.
